MEDICAL ASSESSMENTS AND PA MEDICAL MARIJUANA PATIENT CERTIFICATIONS

THE MEDICAL MARIJUANA CERTIFICATION PROCESS

60 days before the expiration date of your Patient Certification and ID Card, you will receive an email letting you know that it is time to renew your registration.  You must renew the registration prior to obtaining a new certification.

To renew your registration, you will need to do the following:

1. Go to this site to log in to your patient account as a returning user;
2. Click the ‘profile settings’ tab in the blue ribbon at the top;
3. Review all information (Please note: The first name, last name and address must be EXACTLY as it appears on your Pennsylvania driver’s license or state-issued ID card. If you have a change of address card, list that address. Your information must match what PennDot has on file for you.);
4. Scroll to the bottom of the page and click the ‘save changes’ button if you made any changes, and then click the ‘profile settings’ tab again; and
5. Click the blue ‘renew my registration’ button. Follow the prompts until you see the message letting you know your information has successfully validated.

After renewing your registration, you’ll receive an email from the PA Department of Health’s Medical Marijuana Program letting you know that your next step is to obtain a new patient certification, which is where we come in.

To schedule your renewal appointment, please add the Medical Marijuana Certification Renewal Consultation to your cart and check out. Once you prepay, you’ll be sent a confirmation email which will include the link to schedule your renewal appointment.

CANCELLATION POLICY

When you book your appointment, you are holding a space on our calendar that is no longer available to our other patients. In order to be respectful of your fellow patients, please call us as soon as you know you will not be able to make your appointment.

If cancellation is necessary, we require that you call at least 24 hours in advance. Appointments are in high demand, and your advanced notice will allow another patient access to that appointment time.

How to Cancel Your Appointment
If you need to cancel your appointment, please call us at 717-874-8480 between the hours of 9:00 am – 5:00 pm. If necessary, you may leave a detailed voicemail message. We will return your call as soon as possible.

Late Cancellations/No-Shows
A cancellation is considered late when the appointment is cancelled less than 24 hours before the appointed time. A no-show is when a patient misses an appointment without cancelling. In either case, we will charge the patient a 50% missed appointment fee.
